Description of the tattoo you would like to receive: (You can send me inspiration photos via email once I reply to your booking form.)

Location - Where is the tattoo going on your body? (NOTE: If you have exisiting tattoos in the surrounding area, please let me know so I can either request an appropriate photo of the area, or we can schedule a time for a consultation for me to take photos and/or trace the blank area.

Size - Approximately how big will the tattoo be? (Use either cm or inch measurements, as well as please describe. ex. "my whole calf" or "half of my forearm under my existing tattoo, approximately 5 inches in length" etc.

Scheduling - Please let us know what days of the week and times tend to work best for you. Please note that weekends book up the fastest, so if you require a weekend appointment, you may need to book well in advance.

Sensory Requests & Considerations: One of the goals with our studio is to make sure all of our clients feel as comfortable as possible during their tattoo appointment. As a neurodivergent team, we have our personal sensory barriers, so we very much understand what it can be like to face challenges in this type of setting. Please let us know below if you have any specific requests for your appointment. Ex. If you'd like to wear headphones or earplugs. Maybe you're shy or non verbal and you'd prefer a silent or semi-silent session. You can bring fidgets, a book, your phone, etc. Maybe you're very touch sensitive. Anything you would like us to know or request, please type in the box below.

Do you have any allergies we should know about before you come to our studio? Food? Adhesive bandages? Second Skin?

*A DEPOSIT IS REQUIRED IN ORDER TO SCHEDULE A TATTOO APPOINTMENT. THIS FEE WILL COUNT TOWARDS THE TOTAL OF YOUR APPOINTMENT. THE PURPOSE OF THE DEPOSIT IS TO COVER THE TIME SPENT ON ADMINISTRATIVE WORK AND DRAWING YOUR DESIGN. ALL DEPOSITS ARE NON REFUNDABLE AND IN THE CASE OF A "NO SHOW" OR A LAST MINUTE RESCHEDULE THAT DOES NOT COMPLY WITH MY 48 HOUR CANCELLATION POLICY, A NEW DEPOSIT WILL BE REQUIRED TO REBOOK YOUR APPOINTMENT.
